Eligibility

You are eligible to enroll in benefits in the first 30 days following your hire or promotion to a salaried position.

Dependents eligible to be covered include:
  • Legal or Common Law Spouse - For a common law spouse, you need to file a notarized Common Law Form with the Benefits Team
  • Domestic Partner - You first need to file a notarized Domestic Partner Agreement with the Benefits Team.
  • Children - Dependent children will be eligible for coverage until their 26th birthday regardless of access to other coverage. This applies even if your child is married, does or does not attend school full-time or lives with you.

Open Enrollment

Open enrollment for 2018 is October 30th - November 13th, 2017. This is your once a year opportunity to review your benefits and make changes to your elections.

During Open Enrollment you can:
  • Add dependents previously not covered without an eligible life event
  • Remove dependents you no longer need covered
  • Make plan changes to benefits for the new year that can better meet your personal and family needs

WHAT'S NEW FOR 2018
  • New Medical Plan: A Consumer Directed Health Plan will replace the High Deductible Plan. The benefits are similar, but the new plan has a few important differences:
    • You will be able to open a Health Savings Account (HSA) to set aside money for out-of-pocket medical expenses now and in the future
    • The annual deductible and coinsurance applies to prescription drugs
    • If you cover dependents, you must meet the full family deductible before the plan will pay benefits
  • Prescriptions: If you are enrolled in an Anthem Medical Plan, to receive benefits for maintenance drugs - ongoing prescription drugs including birth control and medication to control conditions such as high cholesterol or hypertension - you'll now need to purchase them in 90-day supplies either through the mail-order program, a CVS/Caremark pharmacy, or take action to continue receiving a 30-day supply at your local pharmacy.

All other coverage will carry over automatically, but we strongly recommend you review your benefit elections regardless.

  5. Print your confirmation statement after you finish your enrollment
  6. Your benefit elections will become effective January 1, 2018.
