Making Changes

If you have a qualified life event (you have or adopt a child, get married, or get divorced, for example), you can make related benefit changes within 30 days of that event.

Birth

Before your baby arrives
Whether you're a first time parent or growing your family, pregnancy is an exciting and challenging time. The information below is intended to help you navigate this exciting time in your life.

Find out how your benefit providers can assist you during your pregnancy
  1. Visit www.chipotlewellness.com to learn more about Due Date Plus by Wildflower Health, a program designed to help users track their pregnancy milestones, look up symptoms, and address health issues. Participants in wellness who engage in the Wildflower program, will also earn $100 in wellness rewards!
  2. If you are enrolled in Kaiser, you have access to Kaiser's site that has many useful tools and resources to help you each step of the way.
  3. All salaried employees have access to our Guidance Resources ComPsych® Employee Assistance plan, a program designed to help you navigate big changes in your life. Contact Guidance Resources for a free Birth Kit.

Set up your Leave of Absence or Disability pay
Mothers and fathers are eligible to receive 3 paid baby days and additional leave/disability benefits to take time to bond and care for the new baby. It is best to make these arrangements in advance of your leave by contacting the Employee Service Center.

After the birth of your baby
Congratulations on bringing a new child into your family! This is a great time in your life that may bring questions, stresses and changes to your benefits, tax withholding and beneficiary designations. We can help!

Benefit Changes
Chipotle provides a Basic Life Insurance benefit of $5,000 per child. Even if you are not adding the child to medical, dental or vision benefits you MUST add them as an eligible dependent to secure life insurance. To make this change and add your child to other coverage, be sure to follow these steps within 30 days of birth.
  1. Log on to WorkDay
  2. Click on the "All About Me" worklet to begin your life event
  3. Select "Change My Benefits" under "My Benefits"
  4. Choose your event (birth) and enter the date of birth
  5. Upload the proof of birth. Proof of birth can be anything that includes the parent's names, baby's name and baby's date of birth, excluding Social Security cards- you will not be able to proceed until a document is provided
  6. Under the "Enroll Dependents" section of the benefit plans you choose, add your child as a new dependent. Enter all available personal information about your child. SSN may not be available in the first 30 days of birth. Do not wait for it to arrive.If you are enrolled in "employee only" coverage, you will need to change the coverage type in order to add your new dependent.
  7. Submit your event
  8. Your new elections will be effective the date of the birth, once everything is approved by Benefits

Change your W-4 tax withholding
Complete a new W-4 and submit it to your regional People Support Specialist.

Update beneficiary designations
  • Hartford Life Insurance: Log on to The Hartford to change beneficiary designation for your Basic Life Insurance benefit.
  • 401(k): Log onto the Transamerica site or call 800-755-5801. On the site, go to the Account Information tab and select Beneficiary to update your designation.

Adoption

Adoption of a New Child
Congratulations! Adoption brings about lots of changes in your life. The information below is intended to help you navigate this exciting time in your life. If you have specific questions or concerns, please contact the Employee Service Center.

Get reimbursed for 80% of your adoption fees up to $7500 by Chipotle
Fill out an Adoption Assistance Request and send it to the Benefits Team by fax at 303-222-2515 or scan to benefits@chipotle.com. Please be sure to include proof of payment with your request.

Change your address
Log on to WorkDay to adjust your address. Click on the "All About Me"" worklet and select "Edit Your Contact Information" under the "Personal Information" section.

Change your name
Log on to WorkDay to adjust your legal name. Click on the "All About Me" worklet and select "Legal Name" under the "Personal Information" section.

Change your address
Log on to WorkDay to adjust your address. Click on the "All About Me" worklet and select "Edit Your Contact Information" under the "Personal Information" section.

Change your name
Log on to WorkDay to adjust your legal name. Click on the "All About Me" worklet and select "Legal Name" under the "Personal Information" section.

Benefits Changes

  2. Click on the "All About Me" worklet to begin your life event.
  3. Select "Change My Benefits" under "My Benefits"
  4. Choose your event (divorce) and enter the date of divorce.
  5. Upload the proof of divorce - you will not be able to proceed until a document is provided.
  6. Under the "Enroll Dependents" section of the benefit plans you choose, remove your spouse as a dependent.
  7. Submit your event.
  8. Your life event will be effective the first of the month following your event date, once everything is approved by Benefits.

Log on to WorkDay to adjust your W-4 withholding amounts. Click on the "All About Me" worklet and select "Update My Tax Elections" under the "My Pay" section.

Loss of coverage

Loss of Coverage
Loss of insurance coverage can come as the result of many things like a change in job status of a spouse or partner who provided coverage in the past or another change in eligibility for benefits that were being provided. The information below is intended to help you make the necessary changes to your benefits and taxes as well as provide access to resources that may assist you in other ways. If you have specific questions or concerns, please contact the Employee Service Center.

Benefit Changes
To make changes, be sure to follow these steps within 30 days of the event:
  1. Log on to WorkDay.
  2. Click on the "All About Me" worklet to begin your life event. Select "Change My Benefits" under "My Benefits"
  3. Choose your event (Loss of Other Coverage) and enter the date coverage was lost.
  4. Upload the proof of the Loss of Coverage. Please provide a document that shows the termination date of the other insurance coverage. The termination date must be within the last 30 days in order to be approved - you will not be able to proceed until a document is provided.
  5. Make your changes.
  6. Submit your event. The life event will go to the Benefits Team for review.

Change your W-4 withholding
Log on to WorkDay to adjust your W-4 withholding amounts. Click on the "All About Me" worklet and select "Update My Tax Elections" under the "My Pay" section.

Update beneficiary designations

Gain of Coverage

The information below is intended to help you make the necessary changes to your benefits and taxes as well as provide access to resources that may assist you in other ways. Benefit Changes
To make changes, be sure to follow these steps within 30 days of the event:
  1. Log on to WorkDay.
  2. Click on the "All About Me" worklet to begin your life event. Select "Change My Benefits" under "My Benefits".
  3. Choose your event (Gain of other coverage) and enter the date before the start date of your new coverage. (Ex: If your new coverage begins on 8/1, enter 7/31 for the event date. This will end coverage with Chipotle effective 7/31).
  4. Upload the proof of the Gain of Coverage. Please provide a document that shows the effective date of the other insurance coverage. The effective date must be within the last 30 days in order to be approved - you will not be able to proceed until a document is provided.
  5. Make your changes.
  6. Submit your event. The life event will go to the Benefits Team for review.

Common Law Spouse/Domestic Partner

Common Law Spouse/Domestic Partner
Establishing a common law (opposite sex) or a domestic partner (same sex) relationship is a big step and an important change in your life. The information below is intended to help you make the necessary changes to your benefits and taxes as well as provide access to resources that may assist you in other ways. If you have specific questions or concerns, please contact the Employee Service Center.

File a Certification of Common Law Spouse (CCLS) or File a Domestic Partner Agreement (DPA)
Certification of Common Law Spouse (must be notarized), or a Domestic Partner Agreement(must be notarized). Upload a copy during the first step of your life event within 30 days of notarization. Keep the original for your records.

Benefit Changes

Even if you are not adding new dependents to medical, dental or vision benefits you MUST add them as eligible dependents to secure life insurance. To make this change and other benefit changes, be sure to follow these steps within 30 days of the event:
